Postoperative Pseudomonas Aeruginosa Endophthalmitis: Treatment with Imipenem

A 80 year-old-male presented with acute postoperative endophthalmitis. The patient underwent pars plana vitrectomy and intraocular antibiotic injection of vancomycin (1 mg/0.1 ml) and ceftazidime (2.25 mg/0.1 ml). Despite surgery and antibiotics, there was no improvement. Pseudomonas aeruginosa was isolated from vitreous. Since the isolated organism was sensitive to imipenem, intravenous and intravitreal imipenem injection was performed. Two days after the treatment vitritis and anterior chamber inflammation decreased. Over the next month visual acuity gradually improved to counting finger at four meters. Endophthalmitis is among the most serious complications of cataract surgery. Imipenem is effective in the management of endophthalmitis caused by Pseudomonas aeruginosa. Although inflammation can be controlled with prompt antibiotic treatment to which the organisms were sensitive visual prognosis continues to be poor in pseudomonas endophthalmitis patients.
