The Long Term Results of Photodynamic Therapy and Combined Photodynamic Therapy with Intravitreal Ranibizumab in the Age-Related Macular Degeneration

Purpose: To compare the effectiveness of photodynamic therapy (PDT) monotheraphy with PDT combined with intravitreal (IV) ranibizumab in age-related macular degeneration (AMD).
Material and Methods: Fifty six eyes of 55 patients with the diagnosis of choroidal neovascularization (CNV) secondary to AMD between January 2005-January 2010 were included the study. The patients were divided into groups according to the treatment modality. Group I included 20 eyes treated with combination of PDT and IV ranibizumab and Group II included 36 eyes that were treated with PDT. In Group I five patients (25%) had second intravitreal ranibizumab injection. All patients were evaluated prior to the treatment including best corrected visual acuity testing (BCVA), binocular fundoscopy, fluorescein angiography, optical cohorence tomography (OCT).
Results: The mean follow-up was 21.80±11.16 months in Group I and 21.80±11.16 months in Group II. The BCVA before the treatment in Group I and Group II was 0.90±0.40 logMAR and 0.98±0.43 logMAR respectively. The visual acuity was 0.58±0.30 logMAR and 1.18±0.59 logMAR respectively (p<0.05) at 12th month. In 12th months according to pretreatment measurements central foveal thickness was decreased 113 μm in Group I and 120 μm Group II. In 12th months, the mean PDT sessions number was 1.25 in Group I, 1.86 in Group II.
Conclusion: Combination of IV ranibizumab with PDT is more effective than PDT monotheraphy in the treatment of CNV secondary to AMD. Combination treatment decreases the frequency and number of treatment sessions for an improved visual prognosis.
