Prognosis in Idiopathic Epiretinal Membrane and the Association of Optical Coherence Tomography Findings with Prognosis

Purpose: To assess the prognosis and association of optical coherence tomography (OCT) findings with prognosis in patients with idiopathic epiretinal membrane (ERM) after pars plana vitrectomy (PPV)
Materials and Methods: The study involved 38 eyes of 36 patients with idiopathic ERM who underwent PPV between February 2006 and June 2009 in Beyoğlu Eye Research Hospital. Preoperative and postoperative best corrected visual acuities (VA), OCT findings, operative and postoperative complications were evaluated.
Results: The mean logMAR VA was 0.67±0.30 initially and 0.39±0.28 at final examination (p<0.001). VA improved in average of 2.84±4.4 lines. The mean central macular thickness (CMT) was 422±96 μm initially and 292±53 μm at final examination (p<0.001). The preoperative OCT examination revealed focal attachment in 55% and diffuse attachment in 45% of eyes, while intraretinal cyst was present in 10 eyes, pseudohole in 10 eyes and vitreomacular traction in 2 eyes. A positive correlation was found between initial VA and final VA (r=0.418, p=0.019), while a negative correlation was present between initial VA and visual improvement (r=-0.471, p=0.007). A negative correlation was present between final VA and initial CMT, and final VA and final CMT (r=-0.444, p=0.018; r=-0.459, p=0.024, respectively). No correlation was present between final VA and other prognostic factors.
Conclusion: In most of idiopathic ERM cases visual improvement is obtained after surgery. Even if the visual improvement is better with low initial VA, the final VA was worse compared to cases with high VA. Thus, considering surgery at early stages may result in better outcomes.
