RETINAL DETACHMENT SURGERY IN MARFAN'S SYNDROME

Cerrahpaşa Tıp Fakültesi Göz Hast., Ankara Purpose : To study retrospectively, the clinical characteristics and the results of surgical treatment of retinal detachment in Marfan's syndrome.
Methods : Charts and operating room records of 9 eyes of 7 patients with Marfan's syndrome who underwent vitreoretinal surgery for the treatment of retinal detachment were reviewed. Pre and postoperative visual acuity, refraction, biomicroscopy, indirect and biomicroscopic ophthalmoscopy findings (macular status, the extent of retinal detachment, the type and localization of retinal breaks, presence of proliferative retinopathy), surgical procedures, retinal status and follow-up time were obtained.
Results : Mean age of 7 men 35.71 ±10.02 years. Mean follow-up time was 16.42 ± 8.16 months. All the patients were myopic and mean spherical equivalent refraction was -10.66 ± 3.57 D, and had bilateral lens subluxation. The retinal detachment was bilateral in two cases. Retinal breaks were horse-shoe type in 7 eyes, retinal dialysis in 1 eye and giant retinal tear in 1 eye. All the patients underwent pars plana lensectomy, vitrectomy, encirclement and gas or silicone oil tamponading. 2 cases underwent reoperation. Retina was attached completely in 7 eyes and partially in 2 eyes. Final visual acuity was 20/200 in 2 eyes, hand movement in 3 eyes and finger counting in 4 eyes.
Conclusion : The results of surgical treatment of retinal detachment with vitreoretinal surgical techniques in Marfan syndrome show similarity to the surgical results of retinal detachment in non-Marfan patients.
