The Safety and Efficacy of 23 Gauge Transconjunctival Pars Plana Vitrectomy Method in the Treatment of Cases with Idiopathic Macular Hole

Purpose: The aim of this study is to evaluate the safety and efficacy of 23 gauge transconjunctival pars plana vitrectomy (PPV) in patients with idiopathic macular hole (MH).

Material and Methods: Twenty consecutive eyes of 19 patients with idiopathic MH was investigated in this retrospective study. All of the eyes were operated with 23 gauge PPV technique. The cases were evaluated for anatomical and functional (visual acuity) success, and for the intra- and postoperative complications. Optical coherence tomography was used in order to monitor anatomical success and progression. Wilcoxon Signed Ranc test used for statistical analysis. P value of≤0.05 was considered significant.

Results: The mean age was 63.75±8.59 years. Combined cataract surgery was performed to four of 19 phakic eyes. Postoperative 1. day hypotonia (intraocular pressure ‘IOP' <6 mmHg) was observed in 4 eyes (p=0.027). In the other visits, there was not statistically significant differences in the IOP values. Suture was needed for the closure of one sclerotomy port in 3 eyes. Mean follow-up time was 11.10±5.61 months. Anatomical success was obtained all of the cases (%100). Eleven eyes developed cataract progression. There was statistically significant increase in the best corrected visual acuity at the third month and the following visits (p<0.05).

Conclusion: 23 gauge PPV technique is a safe and effective method in the treatment of idiopathic MH surgery. Keywords : 23 gauge vitrectomy, idiophatic macular hole
