Management of Diabetic Retinopaty; Lazer Treatment

Necmettin Erbakan University, Meram Medical Faculty, Department of Ophthalmology, Konya/TURKEY The mechanisms by which lazer photocoagulation mediates its remarkable benefit on diabetic eyes are not fully understood. Several mechanisms have been proposed and all may contribute toward the beneficial effect. Ischemic retina, which produces growth factors, is destroyed, thus reducing the angiogenic stimulus. Another possibility is that retinal cells may produce growth-inhibiting factors or reduce production of growth-promoting factors in response to photocoagulation injury. It is thought that the primary mediating factor is an increase in oxygenation from the choroid to the inner retina that occurs through the lazer scars due to the thinning of the retina in the treated area. Regardless of the relative contributions of these mechanisms, the remarkable efficacy of lazer photocoagulation in the treatment of Diabetic Retinopathy has been thoroughly documented in multiple randomized clinical trials.
