Clinical and Optical Coherence Tomographic Findings in Patients with Acute Posterior Multifocal Placoid Pigment Epitheliopathy

In this study we present clinical, angiographic and optical coherence tomographic (OCT) findings of 7 patients who applied to ophhtalmology outpatient department with the complaint of decreased vision and were diagnosed as acute posterior multifocal placoid pigment epitheliopathy (APMPPE) after a detailed ophthalmological examination. Five of the patients were female and ages ranges from 15 to 44. Four of the patients were in acute period, three were in the remission period. Patients were followed up for 5 days to 9 months. Four patients in the acute period had subretinal fluid formation, one of them had pigment epithelium detachment (PED). In three acute phase patients visual acuity increased to 10/10. Angiograms of patients in the acute period showed multiple hypofluoresencent lesions in the early phase a hyperfluorescence in the late phase. In remission period angiography showed hypofluorescent areas due to affected choroidal perfusion, and hyperfluorescent areas due to window defect. OCT images demonstrated serous retinal detachment, retinal edema, hyperreflectance between inner segment/outer segment (IS/OS) junction and RPE and damage in IS/OS junction in acute period. In later periods, these findings were relatively resolved but in the remission period, irregularities between RPE and choriocapillaris were well defined. In the majority of patients with APMPPE finally there is an increase in visual acuity. Most of OCT findings in the acute phase are resolved in accordance with clinical improvement, also OCT reveals RPE irregularities in remission period.
