The Results of Simulatenous Vitrectomy and Scleral Fixation Technique in Patients with Dislocated Nucleus or Intraocular Lens During Cataract Surgery

Purpose: To evaluate the results of the scleral fixation technique using foldable intraocular lens (IOL) in cases with dislocation of lens nucleus or intraocular lens (IOL) during cataract surgery.

Material and Methods: Three-piece foldable scleral fixation IOL implantation was performed with pars plana vitrectomy in cases with dislocation of nucleus material (5 eyes of 5 patients) or IOL (2 eyes of 2 patients) into vitreous during cataract surgery. Pre-and postoperative visual acuity, anterior and posterior segment findings and postoperative refraction were evaluated.

Results: The mean age and mean follow-up of patients were 73.57±10.24 years (range, 56-87 years) and 10.71±3.35 months (range, 6-15 monts), respectively. The preoperative visual acuity which was 0.04±0.04 (range, hand movement-0.1) increased to 0.16 (range, 0.3-0.8) at the end of the follow-up after surgery (p=0.04). Postoperative mean astigmatism was 2.71±1.59D (1.25-6.00D). Only one patient has minimal tilt of the IOL. No complications were observed during surgery and follow-up period.

Conclusion: The technique of scleral fixation with three-piece foldable IOL is an effective and safe method after complicated cataract surgery and may help to provide good visual results. Keywords : Complicated cataract surgery, foldable intraocular lens, pars plana vitrectomy, scleral fixation
