Choroidal Metastasis Originating From Breast Cancer in a Patient with Diabetic Macular Edema

M.D., Numune Training and Research Hospital, Eye Clinic, Ankara/TURKEY A 48-year-old woman with diabetes mellitus was admitted to our hospital because of the loss of vision in his right eye. Visual acuity was hand motion in the right eye and 0.3 in the left eye. She had a remote history of breast cancer (status post mastectomy) 4 years ago and history of widespread metastatic lesions in the lungs, liver, and kidney for six months. Fundus examination revealed diabetic macular edema in both eyes. Fundus examination of the right eye showed additionally a yellow choroidal mass, measuring about six discs diameter and located below and lateral to the macular area, which was highly characteristic of choroidal metastasis. B-scan ultrasonography showed a plateau-shaped choroidal mass, with inhomogeneous echostructure and high reflectivity. Optical coherence tomography examination revealed a solid mass involving the macular region of the choroid, a dome-shaped elevation of the choroid, fluid in the subretinal space, disrupted integrity of IS/OS junction and intraretinal fluid due to metastatic choroidal lesion or/and diabetic macular edema. Here, we presented a patient with diabetic macular edema and choroidal metastasis. Additional ocular and systemic pathologies should be considered in these patients with diabetic retinopathy and loss of vision. Keywords : Diabetic macular edema, choroidal metastasis, breast cancer.
