VISUAL PROGNOSIS FOLLOWING INTRAVITREAL INJECTION OF TISSUE PLASMINOGEN ACTIVATOR FOR ACUTE CENTRAL RETINAL VEIN OCCLUSION

INTRODUCTION: In this study we aimed to investigate the effect of intravitreal injection of tissue plasminogen activator (tPA) on visual prognosis of patients with acute central retinal vein occlusion (CRVO).
MATERIALS AND METHODS: A pilot study was performed in a consecutive series of 23 patients who had an early onset of CRVO (developed < 21 days with visual symptoms). After informed consent, affected eyes of the patients received intravitreal injection of 65-100µg of tPA. We evaluated the visual results and performed statistical analysis to compare the visual outcome in ischemic and non-ischemic subgroup of patients.
RESULTS: Visual acuity improved to 20/40 or better in 8 of 23 (34%) patients. Visual acuity improved or stabilized in 15 of 23 (65%) cases. None of the patients developed rhegmatogenous retinal detachment, intraocular infection or neovascular glaucoma. There was no statistically significant difference in visual improvement rate between ischemic and non-ischemic group after tPA injection, (p > 0.05)
CONCLUSION: Intravitreal injection of tPA administered early in the course of central retinal vein occlusion may be of benefit to improve the vision without catastrophic side effects.
