³M.D. Asistant GATA Medical Faculty, Department of Ophthalmology, Ankara/TURKEY A 74-year-old male with hyperviscosity due to Waldenström?s macroglobulinemia (WM) presented with bilateral decreased vision. Bilateral serous macular detachment (SMD) was observed in fundoscopy. Optical coherence tomography (OCT) confirmed bilateral SMD, which was associated with choroidal neovascular membrane (CNVM) in the left eye (LE). The patient received plasmapheresis due to hyperviscosity. He experienced visual improvement in the right eye (RE) shortly after the plasmapheresis. However, there was not any visual improvement in the LE three months after the plasmapheresis. Optical coherence tomography revealed the complete resolution of SMD, and also the presence of CNVM with a central foveal thickness of 308 micron. The patient recieved three doses of intravitreal ranibizumab (IVR) injections monthly to treat CNVM in the LE, which later showed no visual and anatomical improvement. We describe the first case of WM with bilateral SMD associated unilateral CNVM in our knowledge who was treated with plasmapheresis and IVR.
