Case Report: Dome Shaped Macula Associated with Foveal Serous Retinal Detachment ? Clinical Follow up and Literature Review

In highly myopic patients, convex protrusion of the macula within a posterior staphyloma, diagnosed by optical coherence tomography (OCT), was described as dome shaped macula (DSM) by Gaucher et al. DSM is often associated with several complications such as serous retinal detachment (SRD), retinal pigment epithelium (RPE) abnormalities, choroidal neovascularization (CNV), all responsible for visual loss. To date it has not been possible to establish effective treatment of this complications.

A 51 years old female presented to our department because of blurred vision in her right eye (OD) in a few years and followed for DSM associated with foveal SRD. The best corrected visual acuity (BCVA) was 6/10 in her OD and 10/10 in her OS. After one year of follow up, BCVA of her OD and SRD remained stable. In 18th month, visual loss in her OD and progression of subretinal fl uid occured. The intravitreal antiangiogenic drug ranibizumab was injected two times a month in her OD. Ranibizumab injection has failed to show effi cacy in follow up period and therefore patient was observed without any treatment. After 8 months of ranibizumab injection BCVA of her OD was 7/10, subretinal fl uid was resorbed and the retina reattached spontaneously and it was confi rmed by optical coherence tomography (OCT). After spontaneous regression of SRD the patient followed about 6 months with same BCVA and any recurrence of SRD. In this case report we aimed to show spontaneous regression of DSM associated with foveal SRD in a patient followed up 3 years.
