LONG TERM RESULTS OF PERFLUOROCARBON LIQUIDS AND SILICON TAMPONADE USAGE IN THE TREATMENT OF GIANT TEAR RETINAL DETACHMENT

This study evaluates the long term results of the intraoperative use of perfluorocarbon liquids (PFC) and the postoperative long term tamponade effect of silicone oil in the treatment of giant tear retinal detachment (GTRD). We treated 18 GTRD eyes of 18 cases. In all cases a pars plana vitrectomy technique was performed associated with transient flattening of the retina by perfluorocarbon (PFC), a long term tamponade was maintained by silicone oil, except in one case where the tamponade was sustained by gas. In these case retinal detachment recurred and silicone oil was used in the reintervention. Encircling scleral buckling was placed in 14 cases and lensectomy had to be performed in 5 of the 12 phakic eyes. After a mean 22.2 month follow up period where silicone oil already been removed. The anatomical success was achieved in 14 eyes (78%), 10 eyes (56%) of which had visual acuity of 20/200 or more. Among the 7 phakic eyes that had not undergone lensectomy, retina was reattached in 5 eyes (71%) with visual acuity of 20/200 or more. However, during the follow up period 2 eyes (29%) developed cataract. Among the 4 eyes that had not undergone encircling buckling where the tears were localized in the upper half, anatomical success was achieved in 3 eyes. The most common cause of postoperative failure in 6 eyes was recurrence of retinal detachment secondary to PVR. Glaucoma developed in one eye and in 2 eyes band keratopathy was observed due to silicone.
In the GTRD cases without PVR clear crystalline lens can be left in place which may act as a barrier for silicone transmission to the anterior chamber and secondly to gain visual and anatomical success without further complications. We also noted that it's possible to obtain successful results in eyes where the tears are localized in the upper half without encircling scleral buckling.
