Anterior segment retinoblastoma, being one of the masquerading syndromes, occasionally cause difficulties in diagnosis. We present a case consulted to our clinic with persistent unilateral hypopyon, who did not respond to the topical antibiotic and steroid treatment initiated at the external center. Upon detection of a retinal mass in the midperiphery on the dilated fundus examination performed at our center, the approach to diagnosis and treatment of the disease has changed. The eye underwent enucleation and the patient received maximal adjuvant chemotherapy upon detection of high risk factors for pathological examination. Meticulous ophthalmologic examination including dilate fundus examination is of a great importance in a 2-year of child with hypopyon and keratic precipitates for the prevention of morbidity and mortality. Keywords : Hypopyon, retinoblastoma, uveitis
