THE COMPLICATIONS WHICH AFFECT THE PROGNOSIS IN THE PROLIFERATIVE DIABETIC RETINOPATHY AND THE EFFECT OF VITRECTOMY

Purpose : To investigate the effect of vitrectomy to anatomic and jisual prognosis in the complications of severe proliferative diabetic retinopathy.
Methods : 227 eyes of 212 patients from whom 94 were female, and 118 were male,with the complications of proliferative diabetic retinopathy were operated by using vitrectomy techniques.
Results : Of the 227 cases, 87 (38%) had vitreous and/or preretinal hemorrhages, 62 (27%) traction retinal detachments, 55 (24%) progressive fibro vascular vitreoretinal proliferations 12(%5) traction-rhegmetogenous retinal detachments, 11(%5) neovascular glaucoma. Preoperatively in 27 (12%) cases had a visual acuity of counting finger, in 63 (28%) hand movements, in 135 (59%) light perception, in 2 (1%) no light perception. Postoperatively, in 40 (17%) had a visual acuity of 0,1-0,4, in 80 (35%) counting finger, in 55 (24%) hand movements, in 48 (21 %) light perception, in 4 (2%) no light perception. It was found that 23 (10%) cases had iatrogenic retinal tears, 30 (13%) traction-rhegmatogenous retinal detachments, 11 (5%) vitreous hemorrhage, 41 (18%) total, 36 (16%) partial lens opacities, 1 (0.5%) phthisis bulbi.
Conclusion : Since the diabetic vitrectomy provides long lasted visual and anatomic stability, it is advised an alternative method of treatment in the cases to do nothing but observation.
