TERSON'S SYNDROME

In Terson's syndrome a sudden increase in intracranial pressure due to spontaneous subarachnoid hemorrhage or head trauma may result in intraocular hemorrhage. The main cause of subarachnoid hemorrhage ise ruptured cerebral aneurysm. Intraocular hemorrhage usually resorbe spontaneously and the visual acuity become normalized. The most common long-term sequelae that may result in permenant visual deficit is the formation of an epimacular membrane. In this study three cases of intraocular hemorrhage due to ruptured cerebral aneurysm were analyzed. Intraocular hemorrhage is bilateral in one case and monocular in two. Conservative approach is recommended in three eyes. Pars plana vitrectomy is performed in one eye who had bilateral vitreous hemorrhage. All of the cases were followed between 4 to 12 months. The final visual acuity were 0.8 and 1.0. Epiretinal membrane, epimacular membrane or retinal detachment or the other complications due to intravitreal hemorrhage, were not seen in any case.
If the vitreal hemorrhage can not spontaneously be resorbed, it should be followed carefully. Vitrectomy can be recommended in cases with the bilateral or dense unilateral vitreous hemorrhage.
