SELECTIVE INTRAARTERIAL FIBRINOLYTIC TREATMENT FOR ACUTE CENTRAL RETINAL ARTERY OCCLUSIONS

Purpose: To determine the effectiveness of selective intraarterial fibrinolytic treatment which is a new alternative method for the treatment of acute central retinal artery occlusions.
Materials and Methods: Four patients with an acute visual loss starting 7 to 16 hours before were included in the study. Visual acuities were less than counting fingers form 1 meter in all of them. Pre- and post-treatment fluorescein angiography was performed in all of the cases. Coaxial catheter sistem was used to reach the ophthalmic artery via femoral artery and fibrinolytic agents (tPA and urokinase) were infused slowly.
Results: Patients were followed up for 26 months. Visual acuities increased in all cases except one. One case experienced transient hemiplegia starting on second day of therapy which resolved within a week.
Conclusions: Selective intraarterial fibrinolytic therapy is an alternative method for the treatment of acute central retinal Artery occlusion cases especially when applied at an. early stage. Possible complications, however, should be kept in mind during patient selection.
