FACTORS AFFECTING THE OUTCOME OF THE LASER PHOTOCOAGULATION THERAPY IN SYMPTOMATIC RETINAL TEARS

Ege Üniv.Tıp Fak.Hast.Göz Hast.A.D., İzmir Purpose: To evaluate the factors affecting the prognosis of the symptomatic retinal tears with acute posterior vitreous detachment after argon laser photocoagulation therapy.
Methods: Prophylactic argon laser photocoagulation was applied for the symptomatic retinal tears in 50 eyes of 50 patients (28 men, 22 women) between the ages 22 and 76 - years-old (mean 56,36 ±11,4 years). After the laser photocoagulation, correlation between the final succes of the therapy and the age, gender, affected eye, time to reference, visual acuity, refraction, localization, shape and diameter of the break, vitreous traction around the break, presence of the localized detachment and the lattice degeneration, vitreous hemorrhage and number of the breaks were evaluated.
Results: Mean follow-up was 17.5 months (±16.2). Good results were achieved in 40 patients (80%) and conventional retinal detachment surgery or vitreoretinal surgery was required for 10 patients (20%) in whom the retina was detached. Traction on the tear, localized detachment and intravitreal hemorrhage were the factors affecting negatively the outcome of the therapy. Although not statistically significant, the success rate was diminished in lately reffered patients.
Conclusion: In symptomatic retinal tears, traction on the tear, localized detachment and intravitreal hemorrhage were the factors diminishing the success rate of the therapy. Close follow-up in these patients is advised.
