ND:Yag Laser Hyaloidotomy in Severe Premacular Subhyaloid Hemorrhage

Dr. We descibe a case of dense premacular subhyaloid hemorrhage in a patient with non-proliferative diabetic retinopathy treated by Neodymium:yttrium-aluminum-garnet (Nd:YAG) laser. Maximum laser power of 8.5 mJ Nd:YAG laser was applicated, and when a small leakage from subhyaloid hemorrhage into the vitreous cavity was observed the laser application was terminated. Consequent vitreous hemorrhage cleared rapidly and visual acuity increased from hand motions to 20/20 on the 10th day of Nd:YAG laser treatment. Nd:YAG laser hyaloidotomy can be considered as a safe, non-invasive procedure leading to dramatic and rapid increase in visual acuity even in cases with prolonged subhyaloid hemorrhage, preventing the need for pars plana vitrectomy.
