Comparison of Panretinal Photocoagulation (PRP) with PRP Plus Intravitreal Bevacizumab in the Treatment of Proliferative Diabetic Retinopathy

Purpose: To evaluate the effects of panretinal photocoagulation compared with intravitreal bevacizumab plus PRP on visual acuity and fundus fluorescein angiographic (FA) signs in patients with proliferative diabetic retinopathy (PDR).
Materials and Methods: In this prospective study 16 patients with PDR and no prior laser treatment were randomly assigned to 2 groups. In group 1, 10 eyes of 8 patients received PRP in 3 sessions (1 session per week). In group 2, 9 eyes of 8 patients received intravitreal bevacizumab (IVB) (1.5 mg/0.06 ml) and 20 days later PRP was performed in the same manner in 3 sessions. Standardized ophthalmic evaluation, best corrected visual acuity (BCVA) measurement and FA were performed at baseline, 1, 3 and 6 months.
Results: There was no significant difference between 2 groups with respect to age, gender, area of fluorescein leakage or BCVA before treatment. No significant difference in BCVA was observed between groups throughout the study period. The total area of fluorescein leakage was significantly reduced in group 2 compared with group 1 at all followup visits.
Conclusion: IVB remarkably augmented the response to PRP and resulted in accelerated regression of retinal neovascularization but did not have an effect on visual acuity results.
